Signature Assignment: Transforming Healthcare Essay
Write 2500 words addressing each of the following points/questions. Be sure to completely answer all the questions for each bullet point. Now that you have drafted chapter one of your DNP Manuscript, you will consider the long-term plans for your DNP project. You will continue transforming the healthcare system problem you selected using a change theory. The healthcare issue or problem can be at the macro, meso, or micro level.

DNP Project Aim: The aim of this DNP quality improvement project was to implement the Diabetes Self-Management Program (DSMP) using the Summary of Diabetes Self-Care Activities (SDSCA) tool to improve diabetes self-care behaviors among adults with type 2 diabetes within an 8-week period at the practice site.
