Focusing on my practice question, related to Emergency Department nurses participating in educational simulation and the effect it has on sepsis bundle compliance compared to selfguided electronic learning,  I identified more than 18 relevant articles using my search parameters. What became immediately apparent is that research directly linking simulation or insitu training to improved quality metrics, such as bundle compliance, is limited. In contrast, there is a substantial body of evidence on sepsis care, bundle compliance, and the ongoing need for education to support early recognition and treatment.

In reviewing the literature, I relied heavily on preappraised evidence such as reviews, evidencebased guidelines, and care standards. As Bissett et al. (2025) note, these sources require the reader to apply their own appraisal to determine relevance and applicability. A challenge I encountered was that several of the strongest articles were more than five years old. Because sepsis care has evolved significantly, I excluded many of these in favor of more current practice guidelines.

Singlestudy research was more difficult to locate, particularly studies examining simulation as a strategy to improve quality metrics. However, I did find several qualityimprovement projects focused on sepsis care and bundle compliance. While many of these were conducted in ICU or medicalsurgical settings rather than the ED, they consistently reinforced the importance of early identification and timely intervention; key principles that directly support the need for ED nurses to understand and execute bundle elements reliably.

Anecdotal evidence made up a large portion of the available literature. Expert opinion often provides valuable insights from clinical experience, but, as Vatkar et al. (2025) caution, such evidence can be influenced by personal bias and may overstate conclusions. This reinforced the importance of grounding my project in stronger evidence whenever possible.

Overall, the search process was more challenging than I anticipated, mainly because evidence on the simulation’s impact on quality metrics remains sparse. I found myself going down several unproductive rabbit holes before realizing I needed to adjust my search terms and refocus. I’m curious how others navigated similar challenges and what strategies helped them redirect when their searches became too narrow or unproductive.

Reflection on Grading the Evidence for the Practice Question

The practice question guiding my literature review examined whether implementation of a standardized, EHR-embedded behavioral health screening and referral pathway improves depression and anxiety screening completion, identification, and referral follow-through across an integrated health system. Grading the evidence to address this question required careful consideration of both methodological rigor and relevance to real-world, system-level nursing practice.

One of the most significant insights from this process was recognizing that the strongest evidence for this practice question was not limited to randomized controlled trials. Many of the most applicable studies were large cohort analyses and quality improvement initiatives implemented across multiple ambulatory and primary care sites. These studies provided robust data on screening rates, identification, and referral-related outcomes and were well aligned with the systems-focused nature of the proposed intervention. Use of the Johns Hopkins Evidence-Based Practice appraisal tools supported consistent evaluation of evidence quality by emphasizing outcome measurement, implementation fidelity, and applicability to practice rather than research design alone (Bissett et al., 2025).

Use of Search Terms to Identify Evidence Types

Search terms played a critical role in identifying pre-appraised, single-study, and anecdotal evidence. Clinical terms such as depression screening, anxiety screening, PHQ-9, and GAD-7 were most effective in identifying single-study and quality improvement literature. Incorporating system- and informatics-focused terms, including integrated health system, EHR-embedded screening, standardized workflow, and clinical decision support, improved retrieval of large cohort studies and implementation-focused analyses that functioned as higher-level evidence for this practice question.

Pre-appraised evidence was most often identified when searches included terms such as systematic screening, population-based screening, and guideline-aligned care. However, relatively few recent systematic reviews directly addressed EHR-embedded behavioral health screening and referral workflows across integrated systems. This highlighted the importance of broadening search strategies to include implementation and quality improvement terminology to capture evidence most relevant to nursing-led practice change.

Challenges Identifying Pre-Appraised, Single-Study, and Anecdotal Evidence

Single-study evidence was the most readily available, particularly quasi-experimental and quality improvement studies examining nurse- or staff-embedded screening workflows with measurable outcomes. These studies were especially valuable because they reflected pragmatic clinical environments similar to the proposed practice setting.

In contrast, identifying pre-appraised evidence specific to system-wide, EHR-embedded behavioral health screening was more challenging. Many systematic reviews focused on the validity or effectiveness of screening tools rather than on workflow integration, referral processes, or sustainability. As a result, some of the strongest evidence informing this practice question consisted of well-designed cohort studies and longitudinal quality improvement initiatives rather than traditional meta-analyses.

Anecdotal and design-based evidence, including implementation reports and transitional care studies, was readily available but required cautious appraisal. These sources were most useful for understanding feasibility, contextual factors, and implementation barriers rather than for determining effectiveness. Consistent with evidence-based practice principles, such evidence informed workflow design and contextual interpretation without being weighted heavily in outcome conclusions (White et al., 2024).

Reflection on Outcome Definition and Practice Question Scope

Peer feedback emphasized the potential benefit of further operationalizing outcomes such as referral follow-through. While operational definitions are essential during implementation planning and evaluation, maintaining broader outcome constructs during the literature review phase supported inclusion of diverse but relevant system-level studies. This approach is consistent with guidance distinguishing clinical practice questions from research questions, in which broader framing allows comprehensive identification and appraisal of available evidence before narrowing outcomes for implementation and measurement (Bermudez, 2021).

Overall, this experience reinforced that not all practice questions lend themselves to experimental designs or extensive pre-appraised evidence. For system-level, nursing-driven interventions, the most robust evidence often emerges from large-scale implementation studies and quality improvement initiatives. Applying a structured appraisal process enabled identification of the strongest available evidence while remaining aligned with the realities of integrated health system practice.
