Introduction
The assessment system designed by Gordon, meets all the requirements for the realization of an effective nurse assessment, so it is a useful tool for assessment with any model nurse discipline. In my observation in our Miami Lakes community doing a health assessment and providing a pertinent statistical data is our sole goal in this paper. To evaluate the community, it was necessary to use sources such as Florida Health Department website, questions among the residents of my community and my journey through the streets of my community, to evaluate the primary health risks and offer services that help our individuals to achieving autonomy. By using this resource, we were able to identify some of the major problems of the community and perform the vita community diagnosis.
Summary of results
The much of the population of Miami Lakes is composed mostly of Hispanics 81.1%, African Americans 3.3%, Whites 14.4%, other 1.2%., The median family income is $ 67,616, the total number of homes is 10,698, the homes have an average value of $ 371,926, the percentage of homeowners is: 65.4%, and the percentage of tenants is of 34.6.%. The Miami Lakes community is famous for its beautiful homes and wooded streets, additionally, it has an extremely low crime rate (Wikipedia, 2017). The age groups most likely to have health care coverage are 6-17 and 6-17, men and women, respectively. There are several health issues that are affecting the community of which the main problem here is the obesity, high blood pressure, Congestive heath failure, Pneumonia, Diabetes, and Acute myocardial infarct amongst the older adults. Hispanics have higher rates of obesity than non-Hispanic whites. (Florida Health, 2015). Obesity is produced mainly by acquiring bad eating habits and lead a sedentary life, away from physical activity.
The most common major for those earning an Associate’s Degree in Miami Lakes, FL was Registered Nursing. With 99 graduates, Dade Medical College-Miami Lakes graduated the largest number of students in Miami Lakes, FL. In 2015, 39 more women than men graduated from institutions in Miami Lakes, FL. The largest share of households in Miami Lakes, FL have 2 cars, followed by 3 cars. The usual method of travel for workers in Miami Lakes, FL was Drove Alone, followed by those who Carpooled and those who Work at Home. In general, the community of Miami Lakes is a very quiet residential area that has no pre-existing night bars or nightlife sites that can disturb the sleeping patterns of the community. But its high presence of traffic especially in the access to main roads like the I 95 or entrance to the Palmetto highway can generate chaos and alter the stress of its residents.
Using Gordon’s Health Pattern framework, the community’s nutrition was assessed through the nutritional metabolic pattern. The framework help analyzes that even the population of Miami Lakes has a good economic income, many people do not have a good diet due to the lack of time to prepare their food, they usually eat between 1 and 2 times a day, generating food disorders. The lack of exercise in the families of the community increases their risk for conditions such as obesity, hypertension and diabetes. Miami Lakes It has many parks available for the community, which facilitates free access to its residents to exercise and prevent obesity. It is recommended 150 minutes of exercise a week to prevent cardiovascular risks, diabetes and other risks. The barriers that people express about lack of exercise is lack of time, motivation or that they cannot access gym services.
Caring for patients these days importantly must involve the various individuals in the health sector who need to share the information of the patients discussing their management. Thus, with an increase in the use of information communication technologies to keep the health services abreast, even as we have all these exciting techniques people are not ready to engage the technologies for the good of the health sector (E, 2016). Communication systems are essential to solving the community’s health problems. Health professionals can communicate and share what ails the society and why they are not able to solve their problem or at least reduce it to the least levels. There are usually no understandings of the various roles of communication services in the delivery of healthcare services. One of the problems of our community is not taking advantage of the communication channels which should help them be able to fight disease and ignorance.
Leaders are the key drivers of most of the activities in any communities as for when it comes to a decision making they are very much involved. For the community to be able to establish community health and wellness centers to help the people fight chronic health problems. The people on their own cannot be able to create such centers since it will prove expensive and hectic for them, the city count with YMC centers that permit resident maintain a physical health. Some of the positives strength on the community include: Neighborhood with a proud history, intellectual capacity, experience in community development, while some limitations include: Not enough people doing the work, Funders need to become genuine partners. Among the solutions we can offer residents is to promote the use of public parks for exercise and motivate the community to exercise as a family.
Potential barriers to implementing community health plans
Lack of proper will to embrace the various projects that have been initiated by the leadership to solve the problems is a significant barrier to the implementations in the health projects. The solution towards overcoming this problem is by letting the community be involved since the inception of the project such that their input towards the implementation of the plan will be heard.
Conclusion
Many of the families at the Miami Lakes community are at risk of obesity and Diabetes due to sedentary life and lack of time to do exercise. It is necessary to implement policies that regard resources being a barrier to the various proposals that could improve the health sector. Facilitating culturally relevant activities might be reasonable approaches to increasing physical activity (Slattery, Sweeney, Edwards, Herrick, Murtaugh, Baumgartner, & Byers,2006). The way to solve this is to prioritize the health projects in the community allocating enough money for them, also provide counseling to the community about risky behaviors, or how to relive life stress to prevent negative consequences. This will benefit the community in a great deal.
