  1. Implementation Plan element.

These slides describe in depth how the project will be implemented with enough clarity that another leader can replicate the plan.

What – make sure the “What” in this plan matches your “I” in your PICOT question. Include all details.

Who – your “P” in your PICOT.

Why – your priority from your background and evidence

Where – the physical setting in the clinical facility

When – time frame for your implementation

How – details of reliability & validity of tool, cut point if applicable plus measurable outcome description

Cut scores are the scores indicating achievement of the outcomes. For example, the sum of X is at or greater than the target score.

 

  1. Use of Change Theory element.

Include the Name of the Theory and the primary source of the theory citation (the theorist’s words in a source written by the theorist). May need to do a search for a primary source citation.

The rationale for the choice of theory should be clear. Will be clear if we can see the match between what you did (the implementation plan in the context of the environment) and elements of the theory that guided you to make a successful change.

 

  1. Evaluation element.

The Evaluation Plan must relate to the problem and the implementation plan.

Select a QI theory or method, e.g., PDSA framework, to guide the evaluation process. Describe the Quality Improvement principles/methods used e.g. audits, quality process: PDCA (Plan, Do, Check, Act).

 

Evaluation questions are asked to evaluate the project related to the results of the intervention. Example questions are in template and the student provides the answers.

 

Include a cost-benefit analysis. One slide in the template is ready to fill in for the cost-benefit (C-B) analysis. An example Cost-Benefit Analysis slide is included in the template.

 

  1. Conclusion element.

State the conclusion (2-3 sentences or bullets) in relation to the actual and/or planned results. The conclusion is a brief summary of the entire project. Consider the implications of the results and make recommendations for the future that result from the project as a whole.
