Building a Comprehensive Health History

The patient is a 26-year-old Lebanese lady living in graduate student housing. It is important to have a comprehensive and productive interview with the patient. A patient’s illness can be influenced by many factors including social, cultural, environmental, or personal factors. Therefore, an interview regarding these areas is useful (Ball et al., 2019). During the interviews, some techniques that would be used include self-disclosure, expressing empathy, and involving the patient in the decision-making process. These techniques would be used because they allow for open dialogue between the patient and the provider. They build a therapeutic relationship between the healthcare worker and the patient (Sullivan, 2019). As a result, the patient can talk about her condition without the fear of being judged.

A comprehensive Risk & Needs Assessment (CRN) tool could be used. This tool would be used because it has been used to classify youth and to determine risk and placement (Nwachukwu et al., 2018). The CRN is used to ensure that youth are served in the least restrictive placement appropriate to the identified risks and needs. The CRN is periodically used to reassess the youth to determine changes in risk and needs.

The 5 questions that would be asked include:

  • What are your name and age?
  • Do you drink, smoke, or take any hard drugs?
  • Have you had thoughts of killing yourself?
  • What contraceptive method are you using?
  • Is there any cultural practice that you believe to interfere with your access to healthcare services?
  • What can you say about the environment you stay in?
  • Is there anything you would wish to share with me?
