Joshua is a 12-year-old 6th grader living with his grandmother. His behavior has declined since his younger sister joined their household, leading to feelings of jealousy and frequent angry outbursts. He exhibits signs of heightened arousal, such as difficulty sleeping, impaired concentration, and irritability. Recently, Joshua was diagnosed with oppositional defiant disorder and ADHD, though he does not yet have an Individualized Education Program (IEP). Academically, he is at least two years behind in reading and favors Art class over Remedial Reading. When in a good mood, he is talkative and social with peers.
Joshua’s grandmother, who has her own history of childhood trauma, feels overwhelmed by his emotional outbursts and struggles to provide consistent care, including ensuring regular school attendance. She is hesitant to engage with the school, viewing it as an additional burden.
Joshua’s trauma history includes removal from his parents’ care at 18 months due to neglect, physical abuse, and parental substance abuse. An attempt to reunify at age 5 failed after further abuse was reported, leading to his permanent placement with his grandmother. He has limited contact with his mother and none with his father since then.
